Physical Therapist (PT)
Our client, a diversified healthcare delivery network in Cheraw, SC, is currently seeking a Physical Therapist (PT) to join their team. This is a strong opportunity for a PT interested in working with a diverse patient population and collaborating closely with an interdisciplinary care team.
Responsibilities

Evaluate patients and develop individualized physical therapy plans of care based on clinical and functional needs.

Communicate patient progress or problems to supervisor and other team members; assist with patient scheduling and post charges daily to patient records

Provide evidence-based interventions focused on improving mobility, strength, balance, pain management, and functional independence.

Document patient care in accordance with regulatory, licensing, payer and accrediting requirements

Monitor patient progress and adjust treatment plans and goals as appropriate.

Instruct patient’s family or nursing staff in follow-through programs

Document evaluations, treatments, progress, and recommendations in accordance with clinical and professional standards.

Handle job responsibilities in accordance with the Company’s Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standard and applicable state/federal laws
Qualifications

Graduate of an accredited Physical Therapy program

Current Physical Therapy (PT) license in the state of practice or ability to obtain

Current CPR certification
